If you would like to receive the flu shot please contact the receptionist at (801) 802-7373 to schedule your appointment.
We recommend anyone who is pregnant, diabetic, and/or asthmatic to receive the flu shot.
If you do not have insurance the cost of the vaccine at the time of service is:
Ages 9yrs - Adult $22.00
Ages 6 mo. - 8 yrs $26.00
